– The TEL-AML1 fusion in precursor-B ALL is generated by a cryptic 12;21 translocation that is detectable by fluorescence in situ hybridization (FISH). It is generally considered a favorable prognostic indicator. Some TEL-AML1+ ALL patients present at diagnosis with extra copies of the fusion, enumerated by FISH.
Philadelphia chromosome (Ph)/BCR-ABL–positive acute lympoblastic leukemia (ALL) is the largest genetically defined subtype in adult ALL, and until recently the one with the most unfavorable prognosis. Ph+ ALL responds to combination chemotherapy, although complete remission is significantly less likely after standard induction regimens than in Ph−ALL. Combined with a short remission duration, there is a median event-free survival of 8 months; prognosis is poor. Five-year overall survival rates of between 10–20% are typical when treatment is chemotherapy alone.
